Immediate Recurrence in the Hepatic Remnant after Resection of Heepatocellular Carcinoma

Abstract
|
|
|
A case of a widespread multinodular recurrence in the hepatic remnant immediately after re-section of hepatocellular carcinoma is reported.
Hepatocellular carcinoma tends to spread intrahepatically through the portal vein system, and during surgery, manipulation of the liver is unavoidable to mobilize the cancer-bearing he-patio lobe.
Therefore, tumor thrombi in the portal vein present before the operation and / or cancer cell dissemination via the portal vein stream from the operative manipulation seems to be the most significant factor related to early recurrence in the remnant liver after resection.
This experience suggests that it is necessary to establish a non-touch resection technique to prevent tumor dissemination caused by operative manipulation and postoperative adjuvant chemoembolization therapy.
